Elderly and Disabled Waiver Program (EDWP) – is a Medicaid waiver program that provides case-management to help the elderly and/or functionally impaired individuals live in their homes or in assisted living/personal care facilities instead of nursing homes. Find out more about CCSP here.

Non-Medicaid Home and Community-Based Services (HCBS) – provides case management services to qualified individuals who need nutritious meals, homemaker or chore services, caregiver relief, or home modifications.

Wellness Education – provides information about nutrition, physical fitness, and chronic disease self management classes delivered by qualified presenters.

Service Options Using Resources in a Community Environment (SOURCE) – is an enhanced primary care case management program that serves disabled beneficiaries and works to improve the health outcomes of persons with chronic health conditions, by linking primary medical care with home and community-based services through case management.
